Your First Appointment in Three Easy Steps

We’re excited to welcome you to Southern Oregon Gynecology! If this is your first time scheduling with us, follow these three simple steps to get started:

Fill Out the Intake Form

Begin by completing our online intake form. This form gathers your basic information, including insurance, and current health concerns. The form is secure, and all information is kept confidential. If you have any questions while filling out the form, don’t hesitate to contact us for assistance.

Receive a Call to Schedule Your Appointment

After submitting your intake form, one of our friendly staff members will contact you to schedule your appointment. We’re also happy to answer any questions you may have about your visit. Our goal is to make sure you feel confident and prepared before you even step into our office.

Arrive at Your First Appointment

On the day of your appointment, you’ll be greeted by our warm and welcoming staff. We recommend arriving a few minutes early to complete any final paperwork. Our team will be ready to provide you with personalized care and address your health concerns in a comfortable, supportive environment.
